GLP-1 Medications Just Got Affordable: What Medicare’s Bridge Model Means for Seniors
If you’re on Medicare and struggling to afford GLP-1 medications for weight management, here’s something you need to know: a new program called the Medicare GLP-1 Bridge launched on July 1, 2026, that could bring the cost of certain medications down to just $50 per month.
That’s a significant drop for drugs that have commonly cost over $1,000 without insurance. Let’s break down what this program is, who qualifies, and what it means for your healthcare budget.
What Is the Medicare GLP-1 Bridge Program?
The Medicare GLP-1 Bridge is a time-limited demonstration program created by the Centers for Medicare & Medicaid Services (CMS). It allows eligible Medicare beneficiaries to access certain GLP-1 medications at a flat cost of $50 per month.
The program runs from July 1, 2026, through December 31, 2027 — about a year and a half. During that window, CMS is evaluating how these medications affect health outcomes and costs for Medicare beneficiaries, with an eye toward potentially expanding coverage in the future.
The Bridge program operates outside standard Part D coverage. That’s important: it’s a separate pathway designed to give eligible people access to these medications while the longer-term BALANCE Model (which would integrate GLP-1s into standard Part D plans) remains on hold.
Which Medications Are Covered?
The program covers three specific GLP-1 drugs:
- Foundayo (oral tablet)
- Wegovy (injection or tablet)
- Zepbound (KwikPen only — single-dose pens and vials are excluded)
Your cost is $50 per month, regardless of income level. That flat fee doesn’t count toward your Medicare drug plan deductible or yearly out-of-pocket limit, but it’s a predictable, manageable expense for anyone who qualifies.
Who Is Eligible?
To qualify for the Medicare GLP-1 Bridge, you need to meet all four of these requirements:
-
You have Medicare Part D prescription drug coverage — either through a standalone Medicare Drug Plan or a Medicare health plan that includes drug coverage. If you’re in a private fee-for-service, cost contract, or PACE organization plan, you’re not eligible through this program.
-
You’re not currently getting a GLP-1 drug through your Medicare drug plan — if you’re already receiving GLP-1 medication through your Part D plan for any reason, you need to keep using the pathway you’re already on.
-
You do not have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease — if you have any of those conditions, your Medicare drug plan may already cover a GLP-1 drug for you. Contact them directly.
-
You meet at least one of these medical criteria:
- A Body Mass Index (BMI) of 35 or higher
- A BMI of 30 or higher with heart failure, hard-to-control high blood pressure, or chronic kidney disease (stage 3a or above)
- A BMI of 27 or higher with prediabetes or a history of heart attack, stroke, or blocked arteries
Your doctor can calculate your BMI if you don’t know it. As long as you meet these criteria and have Medicare Part D, the Bridge program may help you access treatment.
How Do You Get GLP-1 Drugs Through the Bridge Program?
The process starts with a conversation with your doctor. Here’s what to expect:
- Talk to your doctor about whether a GLP-1 drug is right for you and if you qualify for the Bridge program.
- If one of the covered drugs is appropriate, your doctor sends the prescription to your pharmacy.
- The pharmacy may ask for your Medicare ID number to process the prescription.
- A form is submitted to Medicare for coverage approval. You’ll receive a letter confirming your medication is covered.
- Pick up your medication at the pharmacy and pay $50 for a one-month supply.
Once you’re approved, refills don’t require a new approval as long as you stay on the same drug — even if your dose changes.
Why This Matters for Seniors
For many Medicare beneficiaries, the cost of GLP-1 medications has been a barrier to managing weight-related health conditions. Before the Bridge program, these drugs were largely excluded from Medicare Part D coverage, leaving people to pay out of pocket or go without.
At $50 per month, the Bridge program is making these treatments more accessible. That price point could help more seniors manage conditions like obesity, which is a major risk factor for heart disease, diabetes, and other chronic conditions that drive long-term healthcare costs.
The program also gives CMS valuable data on how these medications impact health outcomes for older adults. If the demonstration shows positive results, it could pave the way for broader Medicare coverage of GLP-1s in the future.
What Employers and Benefits Directors Should Know
For employers sponsoring group plans, the arrival of the GLP-1 Bridge program has a dual impact:
- It signals that CMS is moving toward broader coverage of GLP-1 medications for obesity. This could change how group health plans structure their benefits going forward.
- Employers with aging workforces may find that their Medicare-eligible employees are following Medicare-specific programs more closely — which could influence decisions about bridge coverage or voluntary benefits.
If you’re an HR leader looking at how to manage chronic disease costs for your workforce, the Bridge program is a conversation worth having. At Trek Insurance Solutions, we help employers evaluate GLP-1 HRA solutions that can complement existing benefits — carving out obesity medication carve-outs that reduce costs while improving employee health.
The Bigger Picture: Medicare Is Evolving
The GLP-1 Bridge program is one more example of how Medicare is adapting to new treatments. The BALANCE Model — which would have allowed Part D plans to opt in to GLP-1 coverage — remains delayed, but the Bridge program serves as a bridge (as the name suggests) to a potential future where these medications are a standard benefit.
For now, the Bridge program is the path forward for eligible seniors. If you’re on Medicare and have been waiting for medication access, this may be your opportunity.
