Medicare GLP-1 Bridge: What Seniors Need to Know About $50 Weight-Loss Drugs

If you’re on Medicare and have struggled to afford weight-loss medications like Wegovy or Zepbound, you may have a new option. Starting July 1, 2026, the Centers for Medicare & Medicaid Services (CMS) launched the Medicare GLP-1 Bridge — a time-limited program that makes certain GLP-1 medications available for just $50 per month.

This is a significant shift. For years, Medicare did not cover weight-loss drugs at all. The GLP-1 Bridge changes that, at least temporarily, for eligible beneficiaries. Here’s what you need to know.

What Is the Medicare GLP-1 Bridge?

The Medicare GLP-1 Bridge is a demonstration program created by CMS to test expanded access to GLP-1 medications — a class of drugs that has shown strong results in helping people lose weight and keep it off.

Under this program, eligible Medicare Part D beneficiaries can access certain GLP-1 medications at a flat $50 monthly copay, regardless of income. The program runs from July 1, 2026 through December 31, 2027.

The medications covered include:

  • Foundayo® (orforglipron) — an oral tablet
  • Wegovy® (semaglutide) — available as an injection or tablet
  • Zepbound® (tirzepatide) — available as a KwikPen® only

Importantly, this program operates outside of your regular Medicare Part D drug plan. Your Part D plan does not need to participate or opt in — CMS handles the approvals, claims processing, and pharmacy payments through a central system.

Who Is Eligible?

Not everyone on Medicare qualifies for the GLP-1 Bridge. You must meet all four of the following requirements:

  1. You have Medicare Part D coverage — either through a standalone Medicare Drug Plan or a Medicare health plan that includes drug coverage.

  2. You are not already receiving a GLP-1 drug through your Medicare drug plan — if your plan already covers a GLP-1 for any reason (such as type 2 diabetes), you must continue getting it through your plan.

  3. You do not have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease — if you have any of these conditions, your Medicare drug plan may already cover a GLP-1 for you.

  4. You meet the clinical criteria — you must be at least 18 years old AND have a qualifying BMI (Body Mass Index) or combination of BMI and health conditions:

    • BMI of 35 or higher, OR
    • BMI of 30 or higher with certain types of heart failure, uncontrolled high blood pressure, or chronic kidney disease (stage 3a or above), OR
    • BMI of 27 or higher with prediabetes or a history of heart attack, stroke, or blocked arteries

Your doctor can help you determine your BMI and whether you meet these criteria.

How Does the Process Work?

The enrollment process involves a few steps:

  1. Talk to your doctor about whether a GLP-1 medication is right for you and if you qualify for the Bridge program.

  2. Your doctor sends a prescription to the pharmacy.

  3. The pharmacy verifies your eligibility — they may ask for your Medicare ID number to confirm you’re enrolled in an eligible plan type.

  4. Your doctor submits a prior authorization form to Medicare for approval.

  5. You receive a letter from Medicare confirming your coverage.

  6. Pick up your medication at the pharmacy and pay $50 for a one-month supply.

Once approved, you won’t need a new authorization for refills as long as you stay on the same drug, even if your dose changes.

Why This Matters for Seniors

Obesity is a serious health concern for older adults. It increases the risk of heart disease, diabetes, joint problems, and other chronic conditions. Until now, Medicare beneficiaries who needed GLP-1 medications for weight management often faced out-of-pocket costs of $1,000 or more per month — putting these effective treatments out of reach for many.

The GLP-1 Bridge program changes that equation. At $50 per month, these medications become accessible to a much larger population of seniors who could benefit from them.

CMS Administrator Dr. Mehmet Oz noted: “These treatments are a major medical advancement, but too many seniors are currently unable to access them due to high cost. The Medicare GLP-1 Bridge changes that.”

What Employers Should Know

For employers offering group health plans, the GLP-1 Bridge also has implications. If your workforce includes Medicare-eligible employees, this program may reduce the burden on your group plan for weight-management medications. Employees who qualify can access these drugs through the Bridge program rather than through your group coverage.

This is worth discussing with your benefits advisor, especially if you’re looking for ways to manage healthcare costs while still providing meaningful coverage to your team.

Important Things to Keep in Mind

  • This is a temporary program — it currently runs through December 31, 2027. The future of GLP-1 coverage under Medicare beyond that date is uncertain.
  • The $50 copay does not count toward your Medicare drug plan deductible or yearly out-of-pocket limit.
  • This is not the BALANCE Model — CMS had originally planned a broader BALANCE model that would have let Part D plans opt into GLP-1 coverage. That model has been delayed indefinitely, and the Bridge program has been extended instead.
  • Talk to your doctor first — GLP-1 medications are not appropriate for everyone. Your doctor can help you understand the benefits and risks based on your individual health situation.
